About Investigations Differently
Increasingly, organisations have found that traditional investigation methods, employing a linear framework such as root cause analysis or 5 whys, are unable to consistently identify the true organisational issues. This has led to poor investigation outcomes and ineffective corrective actions such as re-training, disciplinary action etc.
Our Blueline Investigation methodology is based upon the culmination of the research, knowledge and understanding of the leaders in safety thinking and innovation of our time, including Erik Hollnagel, Sidney Dekker, Todd Conklin, Drew Rae and Bob Edwards.
The Blueline Investigation methodology focuses on a ‘Work as Normal’ approach, that combines Safety II with the principles of Human Organisational Performance (HOP), to identify the true organisational issues that contributed to an event.
