ENABLING ORGANISATIONAL CHANGE
WORKPLACE INVESTIGATIONS & RISK MANAGEMENT
We take a fresh approach to workplace investigations and risk management training for high-risk organisations. We enable organisations to learn critical information from their people about the way work is completed every day. We show them how to take this information and translate this into practical measures for improvement.
We offer a full range of investigations and risk management services designed to help organisations to identify the real issues that can contribute to a risk or incident. Our services include tailored workplace investigations training and risk management training (face-to-face and virtual), webinar training, system maturity assessments, coaching and mentoring, and facilitation.
At Investigations Differently, we focus on helping organisations learn from “normal work”. Our approach utilises a combination of contemporary safety theory including Safety I Safety II, the principles of Human Organisational Performance (HOP), Safety Differently and Resilience Engineering. We don’t believe that effective investigations and risk management should be top-down driven and instead our approach focuses on harnessing the worker as the solution.
What is “normal work”?
Normal work is what goes on in organisations every day. It’s a true reflection of the work rather than what is believed to be happening. Errors are part of “normal work” and when they occur our approach is not to blame but to learn the systems and context of the work that enables human error.
We work with organisations to apply this approach to their investigations and risk management to enable real change. Over time, what we see is:
- Improved worker engagement
- Effective investigation outcomes
- Identification of genuine risks
- Systemic risk reduction
- Actions that address the risk, not add to the administrative burden
- Less safety clutter
- Reduced injury severity

Let’s be honest: Freak accidents will always happen. So how do you identify the unknown – let alone assess it?
According to a Policy and Practice in Health and Safety study, 2017, construction organisations with a Zero Incidents policy and perfect safety records are the most likely places a fatality will occur.
The industry has learned nothing new resulting from the investigation. There is no innovation and most concerning of all, there has not been any measurable reduction in risk for these tasks.
Mark and the team at Investigations Differently have had a significant involvement in the evaluation and improvement of key processes within the Mitchell Services business over the past 3-5 years. We have used their expertise in investigations to completely change not only how we investigate, but how we as a business learn from events and from normal work. Investigations Differently significantly bolstered our processes to be more tailored towards an operations and people focus rather than pure compliance, however this work also allowed us to maintain our ISO certification.

Investigations Differently – provided tools to better gain the information required to not only address the issues surrounding an event, but identify systemic issues that can be addressed to prevent similar events from occuring in the furure.
The facilitation was engaing, used real life experiences and examples, to engage students. Practical exercises targeted relevant subjects for the audience, to better meet outcomes.
