Our Investigation Maturity Assessments are designed to provide organisations with strategies that they can implement that will improve their capacity to learn from incidents and deliver a systemic reduction of safety risk.
Investigations Differently provides organisations with independent maturity assessments of their incident investigation process to identify the quality and effectiveness of investigations and investigation outputs to identify improvement opportunities.
The review process involves the embedding of the project team into day-to-day operations to observe normal work and how incident investigations are approached and executed. Our discovery strategy includes:
- A review of the investigation related procedures and documents.
- Interviews with a selection of personnel regarding their perception of and experience with the incident investigation process.
- An assessment of a selection of incident investigation reports against our 10 point Maturity Assessment Framework.
- Provision of a summary document based on the field based work and Maturity Assessment outcomes to deliver observations and recommendations.
During fieldwork, our consultants share industry experience and good practice with frontline personnel to create change and understanding of the critical aspects of incident investigation.
The ultimate intention of this work is to implement strategies that improve the capacity of personnel to learn from incidents and deliver the systemic reduction of safety risk.
